Occasional MS Access fail on start with reference to QlmLicenseLib

After protecting an MS Access application, I noticed that about one time in 10, after opening the MS Access .accdb file, the application does not render on the screen. If I check processes in Task manager, I can see it running, and the ,laccdb file is also created (and can't be deleted because it is used by the Access program). To correct, I end the process in Task manager, and then I can delete the .laccdb file.

To trouble-shoot, I created a blank Access database, and only referenced the file: QlmLicenseLib in VBA. I then opened and closed this .accdb file several times, and was able to reproduce the problem.

Any ideas what the problem is?
